The Hmong American Partnership(HAP) began the Eat Smart, Stay Active/Noj Zoo, Nyob Zoo program. The program is part of the Eliminating Health Disparities Initiative (EHDI) which seeks to create lifestyle changes for participants who are at high risk for being diagnosed with Type 2 Diabetes or who are pre-diabetic. The lifestyle change program is evidence-based following a curriculum developed by the Centers for Disease Control and Prevention.
