During well-child checks, access proven tools to screen, coach, and refer moms to services to ensure they have what they need to improve their health. Discover an innovative way to reach moms and screen them for tobacco use, depression, family planning, and folic acid intake.
Your clinic can do this through the IMPLICIT (Interventions to Minimize Preterm and Low Birth-Weight Infants Using Continuous Quality-Improvement Techniques) grant program. Apply and become part of a statewide effort, which is part of a national endeavor of the Family Medicine Education Consortium, Inc.
Two chosen clinics will receive:
● Free technical assistance to implement the IMPLICIT interconception care toolkit;
● Free IMPLICIT trainings;
● Free support in providing continuous quality improvement to improve women’s health and prevent birth defects;
● Connections to other clinics in your cohort; and
● A $12,500 award over the course of the project.
IMPLICIT improves birth outcomes while promoting the health of women, children, and families. The IMPLICIT Interconception Care Model is a cutting-edge model that embeds care for moms who might become pregnant again into well-child visits. It’s a preventative care model that incorporates assessments and referrals.
