Minnesota Department of Health (MDH)
ACET Inc. was awarded a grant by MDH (2019-2024) to coordinate the Minnesota Statewide Pediatric Care Coordination Community of Practice. The Community of Practice provides a way for care coordinators to network, collaborate, share, and learn from each other to better serve children and youth with special health needs.
The goal of the Community of Practice is to improve positive health outcomes by building the capacity of all systems that serve families of children and youth with special health needs.
